List forms customization stopped working after site url change

We have customized SharePoint list forms using PowerApps (Customize Forms) option. Everything went fine, we were using the customization for few days. 

Then we had to change the site url for some reason.

And after site URL change, list forms stopped working. When we view/edit list item we are getting below error

If I change back the site URL to old one – it starts working again.

But I have to change the site URL to proper one. I cant keep using old URL. There are more than 3 lists on which we have done such customization.

If I try to hit “Customize form” option again to check if it opens in powerapps – we get below error

Let me know if some one can help me with this.
