I have uploaded staff handbook onto a library and will like users to sign off by clicking accept or reject button on a message box displayed whenever they open the file.
My challenge is that there is know option to trigger workflow when an item is open in sharepoint designer 2010.
I need help to make this happen.

You can update the outcomes in a task/approval process to be accept and reject which doesn’t involve any superior. There is not an OOTB way to have a workflow start when the file is opened.
If you start the workflow on the file itself and assign accept/reject tasks to everyone the email they receive will include a link to the document and also a link to the accept/reject task that they perform.
