Adding a title header to each group of questions in using survey template – SharePoint2013

Hi All,

I am working with the Sharepoint Survey form. The requirement was to add a set of questions grouped under specific categories like Official Info, Personal Info etc ., The Grouping can be done with the help of Page-Break which can be used directly by checking the option.
But the problem comes when we have to add a Title-Header for each group of Questions (Which is not available directly in the options).
Any ideas will be appreciated
