Infopath form – Data fetched from SQL issue

I am on SharePoint Online and using Infopath 2010.

I have created a data connection to fetch the data from SQL Server. IT IS WORKING PERFECTLY WHEN IT IS IN PREVIEW MODE, it is throwing an error when i am clicking “Add new item” link. The error is,

A database that contains data required for this form to function correctly cannot be found.

An entry has been added to the Windows event log of the server.

Log ID:5566

Any inputs in helping me resolve this issue
