I created a custom workflow for special request routing. It is to send an email to the creator confirming the request, and an email to the approvers to review, however, yet another email gets sent with no information in it other than “No Title”… Title is not one of the fields that I am using. I created my own.
It’s an automatic email that I cannot find to be able to remove.

Have you checked for errors in the workflow history list?Should be located at a location similar to:
http://[servername]/[sitename]/lists/Workflow%20History
