How to customize SharePoint 2013 workflow task form? Display some additional fields with data for display or collect data from approver.

How are you creating the workflow? With SharePoint Designer or Visual Studio? In SharePoint Designer, if you are creating a collect data step you will create the fields needed for collection and they will display in the task form.
Not sure how you do it in Visual Studio. If you need more details let me know, I’ll type a blog posting with screenshots.
